Gout is disorder of purine metabolism characterised by hyperuricemia due to excess of consumption of food rich in purine results in deposition of uric acid (monosodium biurates ) in cartilage of joints and ear resulting in acute attack of ARTHRITIS.


  1. Gout is commonly seen in males.
  2. Age-35-40 years.
  3. Commonly seen in non vegetarians compared to vegetarians.

Clinical Features:
Gout is classified in two

  1. Acute gout.
  2. Chronic gout.

Acute gout

  • Onset is sudden,pain in joints which is severe,unbearable,agonising type of pain.
  • Pain which aggravates on movement,touch,cold. Pain better by rest.
  • Swelling of joints it looks inflammed along with constitutional symptoms like malaise,fever,anorexia.

Chronic gout

  • Pain in joints which is of prolonged duration.
  • Gouty tophi especially on ear lobe,joints.
  • Deformity of affected joints.


Peventive aspect is to avoid eating foods rich in purine,low purine diet,low protien diet.
Diet-avoid foods which are rich in purine like fish,liver,sweet bread,red meat,meat extracts,meat soups. food like vegetables,fruits,milk,milk products,eggs,fats,sugar,sweets and cereals are low in purine content. plenty of water,avoid alcohol intake. local heat or cold to relieve the pain.


CureFactor for Osteomalacia


Osteomalacia is deficiency disorder of vitamin D especially seen in adults(in children’s it is called RICKETS). It is characterised by bone pain,tenderness,proximal muscle weakness,waddling gait and tetany.


  1. Low intake of food rich in vitamin D.
  2. Inadequate exposure to sunlight.
  3. Commonly seen in adults and females are more effected.

Clinical Features:

Symptoms : The time of onset of osteomalacia is insidious takes time to appear. First thing to be noticed is pain in bone especially dull type of pain well marked in back,thigh,lower limbs. Pain usually worse night time. Muscular weakness,anorexia,weight loss,spontaneous fractures are common.


  • Tenderness present.
  • Proximal muscle weakness.

General Management:

  1. To educate the patient to have diet rich in vitamin D food like egg,milk,meat…etc and also diet rich in calcium. Vitamin D helps calcium absorption so intake of calcium is also must.
  2. The cheapest means of vitamin D is sunlight,daily exposure at least for 10 mins morning time will be helpfull

Adequate exposure to sunlight,eating food like cod liver oil,fish,milk,egg yolk,butter which are rich in vitamin D. Daily requirement of vitamin D is 400IU(International units).
CUREFACTOR: Avoid intake of cereals it interferes with vitamin D absorption.


CureFactor for Emphysema



Emphysema is condition characterised by dilatation of alveoli(situated within the lung cavity) and over aeration of alveoli,with loss of pulmonary elasticity and function,is usually a complication due to long standing chronic bronchitis.


  1. Predisposing causes of emphysema are
  2. Chronic bronchitis
  3. Smoking
  4. Bronchial asthma
  5. Occupational hazards : Occupation which requires exceptional forced breathing like glass blower band playing wind musical instruments
  6. Occurrence of emphysema is common in males.

Emphysema is pathological condition start off with upper respiratory infection lead into lower respiratory infection leading into decreased functioning of the lung capacity and lung volume.

Clinical Features:

Initially emphysema is asymptomatic( no symptoms).
As the disease progresses the first symptom noticed is dyspnoea. Dyspnoea which is progressive increasing in severity over several years initially on exertion later5 on even during rest.

  • Cough which is worse in the morning.
  • Expectoration(discharge of cough) is copious in the morning later on it becomes mucoid or mucopurulent(infection)
  • Gradual weight loss and weakness signs
  • Central cyanosis,clubbing of nails and respiratory rate hurried.
  • If left untreated leads to complication like acute respiratory failure,spontaneous pneumothorax and recurrent chest infections.

General Management:

  • The underlying cause to be treated
  • As a rule stop smoking
  • Keep yourself warm,avoid exposure to cold weather.
  • Regular breathing exercises.
  • If occupation is the cause,take a break from job

Chronic Bronchitis:

Prolonged and repeated attacks of acute bronchitis resulting in cough with expectoration,dyspnoea on exertion, bronchospasm.
criteria to call it chronic bronchitis.
Above mentioned symptoms at least for three consecutive months in a year for more than two successive years.


Predisposing cause for chronic bronchitis:

  • Smoking as a rule is the main cause for majority of respiratory diseases

  • Familial predisposition is usually positive.

Clinical Features:

Any chronic disease takes time to appear hence chronic bronchitis has a very insidious onset(slow to appear)
symptoms aggravating with either exposure to cold weather(winter) or smoking brings about cough is first thing we notice in case chronic bronchitis. The character of cough in chronic bronchitis is mucoid or mucopurulent. Dyspnoea on exertion is marked feature of chronic bronchitis.fever is usually absent and only present during an acute attack.
chronic bronchitis if left untreated can lead into complications like emphysema(distension of alveoli),bronchiectasis,mechanical cause due to long standing cough hernia(protrusion of internal content to outer surface) can occur.

General Management:

  1. Stop smoking if smoking is the cause for the disease for early recovery.
  2. Avoid eating foods served cold,avoid drinking cold water or anything that is cold is to be avoided.
  3. Keep yourself warm
  4. Start drinking warm water.
  5. Steam inhalation for temporary relief
  6. Regular early morning breathing exercises will be helpfull
  7. If occupational hazard is the cause simple precautions like wearing masks working in ventilated place will avoid the disease occurrence.

  2. Frontal sinuses.
  3. Ethmoidal sinuses.

  3. Hypertrophied turbinates.
  4. Exposure to cold can cause an attack of sinusitis
  5. Excessive blowing of nose during an acute attack can causes sinusitis.

Clinical Features:

  • As a general rule,situation of pain helps in knowing which group of paranasal siuses are inflammed.
  • Pain around the cheek and infra orbital region(maxillary sinusitis)
  • Pain around the frontal region frontal headache( frontal sinusitis)
  • Pain around the nasal bridge and behind the eyes(ethmoidal sinusitis)
  • Nasal discharge is usually yellowish green in colour,pain from stooping,post nasal drip(hawking of discharge from mouth) which irritates the throat.
  • Tenderness over the affected sinus on applying pressure, best way to know if you have sinusitis or not is by bending head downwards if you feel the head to be heavy might suggest that you are suffering from sinusitis.

General Management:

  1. Rest in warm room,keep yourself warm.
  2. Steam inhalation should be helpful to relieve the nasal block.
  3. Prognosis is good recurrence is very common.

Acute Bronchitis :

Acute Bronchitis is defined as inflammation of the mucosal lining of bronchial tubes.
Acute bronchitis is characterised by fever,purulent expectoration and loud rhonchi(heard through sthethoscope).
bronchial tubes are the passage of air into respiratory tract that conducts air into the lungs.


Acute bronchitis is lower respiratory tract infection mostly caused due to untreated upper respiratory tract infection(URTI) like sinusitis,laryngitis..etc.
besides the untreated URTI,common causes of Acute bronchitis are viral infection(hemophilus influenza) and bacterial infection(pneumococcus,staphyllococcus aureus)
weather changes like cold damp weather,dusty weather.
predisposing causes like complicating other disease like measles,pertusis.

Clinical Features:

  1. Constitutional symptoms like fever(38-39 celsius),malaise
  2. Cough expectoration is initially scanty,difficult and mucoid as it progresses becomes copious,easy and mucopurulent.
  3. Sore feeling behind the sternum which is worst on coughing
  4. Tightness of chest.
  5. Dyspnoea(difficulty in breathing)
  6. Wheezing.

The progress of the disease(prognosis) is usually good,mild attacks last for 7-10 days where in persistent early morning cough remains for another 1 week, severe attack lasts for 3-4 weeks.

General Management: 

  1. Bed rest
  2. Steam inhalation.
  3. Avoid exposure to cold as it may aggravate the attack.

Deviated Nasal Septum (DNS)

DNS is condition characterised by deviation of nasal septum either to right or to left side of the nasal passage causing mechanical obstruction. DNS is mechanical condition that is if severe requires to operated.


DNS exact cause is not known,few predisposing cause nay seem to be the cause which are as follow.

  • Age:usually most of individual suffer from mild DNS which doesn’t cause much problem to them,and adolescence usually is common due to growth variation and is believed to be normal and doesn’t cause much problem. commonly occurs more in males.
  • Trauma- direct blow over the nose can cause DNS.
  • High arched palate
  • Familial predisposition present.


  1. The first thing that you notice is frequent nasal obstruction which is either continues or intermittently.
  2. Headache and heaviness of head due to nasal blockage.
  3. Recurrent attacks of rhinitis and sinusitis.
  4. mouth breathing.
  5. Dryness of mouth and pharynx
  6. epistaxis in case of trauma over nose.

DNS is classified into two types

  1. C-shaped: deflection of the nasal septum to either right or to the left and causing unilateral block.
  2. S-shaped: deflection of nasal septum to both the side causing alternative unilateral block.
  • DNS if not treated early leads to complications like chronic sinusitis and allergic rhinitis.
  • DNS is mechanical obstruction and requires to be operated surgical. medication can only relieve the complaint temporarily.


Conservative treatment that is medicines are given to just relieve the obstruction.
severely DNS is advised for surgical operation.

Chronic Laryngitis:

Persistent inflammation of larynx causes long standing hoarseness of voice.


Recurrent attacks of acute laryngitis overuse of voice especially seen in teachers,public speakers and is one of the occupational hazard. smoking is one of the predisposing cause of chronic laryngitis allergic attack, few individuals are susceptible to certain allergens like dust,pollens…etc, and repeated exposure can lead into chronic laryngitis. other upper respiratory tract infection like chronic sinusitis can cause repeated attacks of laryngitis leading into chronic laryngitis.

Clinical Features:

Long standing hoarseness of voice is the characteristic symptom of chronic laryngitis. change in the quality of voice,spasmodic cough with light expectoration,sore throat,foreign body or irritation sensation in the throat followed by difficulty in swallowing.

if left untreated chronic laryngitis can lead to complications like total loss of voice(aphonia).

General Management:

Rest to the voice,avoid talking and rest your voice will be helpfull in early recovery. If smoking is the cause or the triggering factor stop smoking. Steam inhalation. Avoid cold things be it in form of food or drinks. Start with warm water immediately.

Acute Laryngitis

Acute inflammation of mucosal linings of larynx which is characterised by sore throat and hoarseness of voice.


Cause of laryngitis is due to bacterial infections and viral infections. most of time the cause for laryngitis is exposure to dust,fumes,dust pollutants…etc enters through mouth and first thing to be affected is the larynx. immunity plays a positive role,usually people who have stronger immunity rarely suffer from laryngitis.

next thing that can cause laryngitis is exposure to cold,damp weather and also drinking icy cold water can cause inflammation of larynx.
apart from the external causes mentioned,one of the most important internal cause is overuse of voice which cause laryngitis(usually chronic case).

Clinical Features: 

The two most characteristic feature of laryngitis is sore throat and hoarseness of voice. Other symptoms are tickling in the throat,sensation of lump in the throat,aphonia(loss of voice),paroxysmal dry,irritating cough and difficulty in swallowing.

General Management:

  • Acute laryngitis usually requires precautions.
  • Early morning gurgles will be helpful
  • Give some rest to your voice avoid talking.
  • Steam inhalation
  • Avoid cold and hot spicy foods and drinks.
  • Don’t allow your throat to dry up keep sip warm water in between.

Allergic rhinitis:

Allergic rhinitis is hypersensitive reaction of nasal mucosa to certain types of substance that results in nasal congestion.

Cause :

Allergic substances are of two types

Exogenous(substance that are around us)

Onset is sudden,naslal itching,violent paroxysmal,profuse watery nasal discharge.
itching and irritation in the eyes.
headache some times.

  • Chronic type(lasting for a longer time period with recurrence of acute symptoms very often)

persistent nasal stiffness
excessive muciod discharge
occassional episodes of sneezing and watery discharge from nose.

General Management:

Identify the allergen and try stay away from it or atleast lessen its use in daily life.
